How to Apply

Step 1 . Determine eligibility requirements.

Please first review our eligibility requirements and funding priorities to determine if your organization, program, or project is a match. Again, we encourage you to contact us to discuss your ideas. Applications are due to IFMC September 30.

Step 2. Download forms.

Please download the following forms:

Budget Form

Budget Justification Form

Step 3: Complete Application and Budget.

Please complete the forms as requested. If you have any questions during this process, we encourage you to contact us with questions.

Step 4: Additional documentation.

In addition to the application, budget, and budget justification, you will also need to provide the following documents in the following formats MS Word, MS Excel, JPEG or Adobe PDF:

  • IRS determination letter regarding 501(c)(3) status (except if governmental entity)
  • List of board members and their affiliations
  • Most recent audit, 990 form, or organization financial statements (only one is required)
  • If applicable, up to two (2) letters of support (not required)

Step 5: Submit a full application.

Should you not provide support letters, the application will submit just fine without them.
