Oklahoma Health Care Authority Grants Program to Iowa Foundation for Medical Care

Oklahoma City, Oklahoma – The Oklahoma Health Care Authority recently announced the award of the SoonerCare Health Management Program to the Iowa Foundation for Medical Care. The contract focuses on improving participating member outcomes and reducing long-term spending. Operation of the program began Feb. 1, 2008.
IFMC will assist high risk and high cost members and their providers by successfully implementing a health management program. The program ensures high quality appropriate care to participants with chronic or episodic health concerns. IFMC’s nurse care managers will provide education and support to coordinate care, improve self-management skills and increase member awareness and use of community resources.
“We are looking forward to working with the state of Oklahoma,” says Mike Speight, senior director of operations for IFMC. “We have experience working with programs similar to this program, and are eager to make a difference in the lives of Oklahoma residents.”
Practice facilitators from IFMC will work with selected physician practices to provide education and technical support for quality improvement activities including data collection and analysis. Quality conferences and monthly collaborative training sessions will be available to SoonerCare Choice PCP Practices statewide. IFMC’s provider education offerings will work to improve chronic and preventive care services for SoonerCare members as well as build strong community partnerships.
